Want to switch to us?
Indiana waiver members can choose and change the provider of any service at any time. Tell your care manager or care coordinator that you choose Journee of Life Home Care. Once they update your service plan, we schedule your first visit.
Not on a waiver yet?
The State of Indiana decides who is approved for Medicaid and for a waiver. We can't approve anyone, but here is where to start:
1. Call your local Area Agency on Aging and ask for a level of care screening. Find yours at in.gov/fssa/da/area-agencies-on-aging
2. Apply for Indiana Medicaid if you don't already have it.
3. There may be a waiting list. The State mails you a letter when your name comes up.
Once you are approved, call us at 1-800-693-0710.
